Coming in 2013!  HHH in coordination with experts from our local medical community is developing a new series of workshops concentrating on how patients and their caregivers can decrease the need for hospital re-admissions and improve overall patient outcomes! 

How To Be A Smart Patient

Highlighting the importance of staying informed and involved in one’s medical care, this program offers tips and information that will help patients and their families become more engaged and proactive with regards to healthcare decision-making. Discussion will focus on important questions patient’s should ask health care providers when faced with a new diagnosis or while undergoing a procedure or medical treatment; how a patient can participate in their care while hospitalized; and how to prepare for discharge from the hospital after a procedure or surgery.  Following the presentation, there will be a brief period for questions and answers.

Health Insurance 101

This program will explore topics related to health insurance, including tips for how to most effectively manage and utilize health insurance benefits, how to learn about health insurance plans, and who to talk about questions or concerns about coverage.  Our presenter will also offer information about Medicare, the new Part D prescription coverage, as well as the regulations and criteria that must be met to qualify for SSI and Disability programs and how to go about enrolling for these benefits.  Following the presentation, there will be a brief period for questions and answers.

Self-Care for the Caregiver

Critical information about caregiving, the stress inherent to the caregiver role and how caregivers can take care of themselves are highlighted in this program.  The goal of this program is to provide caregivers with resources and information that will help them provide support to their loved ones without forgetting the importance of caring for themselves.

Navigating the Medical Maze

This presentation provides information about hospital structure, staffing and specific roles of staff in patient care and management.  The goal of this program is to provide patients and families with the knowledge they need to become active and informed participants in their healthcare by familiarizing them with the hospital environment and internal structures.
